Heaven didn't have either of those. Though, there was one area with the ocean where we were able to walk and explore into the sea without needing to breathe.

[Does he even need to breathe honestly, he's such a creature. He would like being able to walk around in the sea, though.]
The achievements are separate from the competition to Australia, as far as I've heard. If they were the same, my team certainly would have topped the chart.

Especially since over-the-top murders are more likely to get achievements, at least as far as I can tell. [THE COFFINS THOUGH.]
It seems there are quite a few repeat areas. Hell didn't have a morgue, but did have an aquarium.
